Re Entry Permit - TM.8

Hello group, i'd just like to be prepared.  I'm planning on applying for a Single Use / ReEntry Permit.  Online it reads i'll need to complete a TM.8 form.  Unfortunately all the links seem to be dead ends.  Just wondering if I can find one online or if it's simple enough to fill out once I arrive at the office?  

I plan on going to the office at Chang Arena. It gives me more time to have it done than being rushed at the BKK airport.

However, last time I went to the main Buriram Immi office  (not Chang Arena) I was told a form was not needed as it is all done on their computer.

That might be why I can't find it online.  They removed the form, but let it in the "required documents" area.  I'll find out next week.    If there is anything to add, I'll post the results here.

You were a big help my first time around.  I didn't this form that time though.  We talked about the retirement visa and what option was best for me as I return to Canada each summer.  At that time I was on a one year multiple entry from Canada.  So as long as I return to the Kingdom before mid Oct this year, the Re-entry will be perfect.
